The Effect of Clofibrate on Glucose Tolerance in Non-Insulin-Dependent Diabetes Mellitus Patients


Abstract
Insulin-stimulated glucose uptake is markedly impaired in patients with non-insulin-dependent diabetes mellitus (NIDDM), and this defect causes the hyperglycemia and the hypertriglyceridemia and may have an important role in the pathogenesis of
NIDDM.
The goal of this study was to investigate the effects of clofibrate on glucose tolerance in subjects with NIDDM. Clofibrate (1.5g/day) or placebo was administered to 15 patients and an oral glucose tolerance test (OGTT) was performed before 12
weeks
after treatment.
In clofibrate-treated group the blood glucose levels were significantly improved at all time points during OGTT. However there was no changes in insulin invels. After 8 weeks of treatment with clofibrate, fasting glucose levels improved. Insulin
binding
to erythrocytes demonstrated no significant changes in clofibrate treated subjects.
Serum triglyceride levels were significantly decreased after 12 weeks of clofibrate treatment, but serum cholesterol levels were not decreased significantly.
These results suggest that clofibrate improves glucose tolerance in NIDDM subjects and that clofibrate increases insulin sensitivity through an unknown postreceptor mechanism.
